bottom pic shows the back of the card. sorry it’s not clear (both not clear). i’m really an idiot when comes to taking photos. dunno about the lighting. the florescent lamp is behind me… guess the reflection of the light get caught in the photo. i want to test out my digicam macro mode, that’s why one of the reason to post the donor card. ok, the back of the card, top says ‘i, in accordance with the human tissue act of 1974, do hereby promise to donate the following organs upon my death:

re: the blood group, can you believe it, at that time they told me my blood group was group O but several months later when i went to the doctor and did some blood testing, found out my blood is group A. ayoh. like that kind of mistake they also can made?? so i cancel the O there and put in A.
